The results of the Russian non-interventional (observational) data monitoring program for the drug recommendation and prevention of hypovitaminosis and mineral deficiency «PROGNOSTIC» in pregnant women

Abstract

Provision of the body of a pregnant necessary for normal functioning of micronutrients is the most important factor contributing to the physiological pregnancy and normal development of the fetus. The purpose of the study lies in the assessment of the effectiveness and safety of vitamin and mineral complex Vitrum Prenatal Forte in the prevention of hypovitaminosis and mineral deficiency in pregnant women in order to improve maternal and perinatal outcomes. Observational program called “PROGNOSTICS” was conducted from September 2013 to January 2016, in 33 cities in 7 regions of Russia. Materials and methods. Clinical and laboratory examination of women, the analysis of the state of the newborn at birth and during the early neonatal period. All patients received vitamin-mineral complex (VMC) Vitrum Prenatal Forte - 1 tablet per day orally, after breakfast. The drug intake was started by pregnant women from the time of registration and continued until delivery. Average duration period lasted for 32 weeks. Results and conclusions. Regular intake of balanced VMC, and in particular, the Vitrum Prenatal Forte promotes physiological pregnancy, reduces the risk of anemia and the incidence of other complications of pregnancy, and their components contribute to proper growth and development of the fetus, contribute to the birth of healthy offspring. It was confirmed and no possible influence was noted on the development of ICH fetal macrosomia.

The paper presents the results of systematic analysis of clinical effects of calcium important during pregnancy, including the appropriate fundamental molecular and physiological mechanisms of the clcium action. Clinical trials show that higher intakes of calcium and vitamin D3 in pregnancy help in the prevention of preeclampsia, normalization of the blood pressure and in the reduction of the risk of dental disease.

Age-associated features of recovery and maintenance of the vaginal microbiota in the therapy of the female genital tract infections

Abstract

Vaginal biotope is a dynamically changing in structure microecosystem depending on the woman's age, menstrual cycle, pregnancy and other factors. In reproductive age the most common infections of the female lower genital tract are bacterial vaginosis, vulvovaginal candidiasis, aerobic vaginitis, mixed infections and sexually transmitted diseases. In post-menopausal period urogenital disorders form in genitourinary syndrome. Independently of age urogenital infections accompanied with damage of vaginal epithelium and decreased lactobacilli level. In a brief review is discussed the rationale pathogenetic therapy aimed at restoring the vaginal epithelium and maintain the growth of lactobacilli in the vagina. Data for the efficiency use vaginal tablets containing Lactobacillus acidophilus KS400 estriol and 0.03 mg per tablet (for 6 days in the reproductive years and at least 12 days - in postmenopause) for these purposes are presented.

Evaluation of mental and emotional state of patients with polycystic ovary syndrome on the background of hormonal contraception

Abstract

Goal. The study of changes in mental and emotional state in patients with polycystic ovarian syndrome before you start taking a hormonal contraceptive containing estradiol valerate (E2V) in combination with dienogest (DNG) in dynamic mode and after 12 months against a background of continued administration. Materials and methods. The study included 36 patients in need of contraception (aged 22 to 31 years), with the presence of symptoms of hyperandrogenism and menstrual irregularities. All patients completed self-assessment questionnaires for mental and emotional state before the reception and at 12 months in patients receiving hormonal contraceptive. Results. Hormonal contraceptive (E2V in combination with DTG) used in a continuous dynamic mode, has a positive therapeutic effect against clinical manifestations of hyperandrogenism and reproductive status of patients with PCOS. The therapeutic effect in combination with E2V dienogest was to authentic decrease in severity of symptoms of hyperandrogenism (acne, fat skin). In addition, in patients receiving hormonal contraceptive it noted improvements in mental and emotional state of patients with polycystic ovary syndrome, which is caused by a decrease in symptoms of hyperandrogenism, and the normalization of body weight (by diet).

Abstract

The combination of various gynecological diseases and pregnancy always carries the risk of aggravated development of the latter, in particular it is accompanied by a higher rate of early reproductive losses. In patients with uterine myoma high risk of premature interruption of pregnancy is primarily due to placental insufficiency - it is especially true for women with submucosal growth nodes. In the presence of submucous node removal of the dead cells of the ovum may be complicated by difficulties in emptying the uterus, uterine contractility in violation of postabortion period, as well as the patient requires referral to the second stage of treatment in the amount of hysterectomoscopic, laparoscopic myomectomy or laparotomic. In this case report, we present an example of the treatment of patients with non-developing pregnancy with submucous uterine fibroids by simultaneously performing vacuum aspiration with hysteroresectoscopy.

Comparative characteristic of eutopic and ectopic endometrial cells in patients with endometriomas

Abstract

Objective. To study the morphometric characteristics of eutopic and ectopic endometrial basal cells in patients with endometriosis and without. Material and Methods: The study involved 76 women aged 17 to 42 (average age was 28.97 ± 5.34 years). The main group consisted of 61 women with ovarian endometriosis, control group - 15 patients with infertility, not related to endometriosis. Results. Morphometric evaluation basal endometrial cells revealed that in patients with endometriosis compared with patients without a thickness of 1.25 times lower than the cytoplasmic membrane thickness gland - 1.5 times the thickness of the nuclear membrane - 1.2 times higher average brightness - 1.2 times lower average density - 1.2 times higher. When comparing eutopic and ectopic endometrial cells in endometriosis patients with the following data: the core area is 1.7 times higher in the eutopic endometrial cells, secretory thickness of the cage 2-fold higher in the eutopic endometrial cells, cytoplasmic membrane thickness is 1.3 times higher eutopic endometrial cells, the average density is 1.2 times higher in the cells of ectopic endometrial average brightness 1.19 times higher in eutopic endometrial cells. Conclusion. The results of our work have demonstrated the obvious statistically significant differences in the morphometric analysis of the basal epithelial cells of eutopic between female patients with endometriosis without it. Consequently, not only stem cells from different ectopic foci localization eutopic cells, but the cells themselves and the basement membrane, which are located on mesenchymal stem cells on their morphometric characteristics are distinct and different from endometrial cells without endometriosis patients.
