The role of pH-metry in the diagnosis of vaginal infections

Abstract

The purpose of the study. Study the clinical significance of pH-metry for diagnosis of bacterial vaginosis (BV) and evaluate the optimal values of the normal micro ecology of the vagina. In addition, the objectives of the study included a comparative study of different methods and indicators of pH-metry. Materials and methods. 214 women aged 18 to 45 years (mean age 30,2±7,7 years) were examined, of which were formed two representative groups. The first group consisted of 114 patients with BV, the second - 100 healthy women selected in the comparison group. Each test was performed on six females pH measuring the different test methods or systems, only two groups - 1284 pH measurements. Comparison was made between foreign and domestic pH test strips on a new modification Colpe test pH.Results and discussion. In BV patients, the pH of the vagina on the measurement ionometers average equaled 6,49±0,07 and 6,24±0,034 depending on modelipriborov. In healthy women, vaginal pH content was within the average was 4,21±0,022 and 4,31±0,041 (p<0,001). Strips of Colpe pH test increments 0,2-0,3-0,5 division were the most effective, because oniobladayut high sensitivity (90%) and specificity (85%) in the measurement of pH in patients with BV. Received findings indicate a high diagnostic value of pH-metry BV algorithm diagnosis of vaginal infections.Conclusion. pH-try vaginal fluid stepper division 0,2-0,3-0,5 is important and must express method to immediately get objective information about the acid-base status of the vagina. Russian plastic strips Colpe test pH can be used to assess the acidity of vaginal fluid to meet all the necessary conditions for the qualitative diagnosis (sensitivity - 90%, the specificity - 85% BV) and must further be used in gynecological practice.

Non-specific vaginitis treatment and its efficacy evaluation

Abstract

Aim of this study was to evaluate the effectiveness of treatment of non-specific vaginitis and clarifying the availability of microbial resistance to therapy of the local Terzhinan combination drug used in practice to over 15 years. The study involved 50 patients who received Treatment based on Terzhinan in 1999 (retrospective analysis), and 50 patients' treatment which was carried out in 2013 ( prospective study). Terzhinan use in the treatment of nonspecific vaginitis shows good clinical effect and tolerability that allows recommend it for the relief of symptoms.There was no formation of resistance to the components of the drug. Terzhinan remains popular and highly effective means of for the treatment of nonspecific vaginitis, which is confirmed by the results our study.

First clinical-morphological tendencies of the treatment ofpatients with myoma of womb with ulipristal acetate

Abstract

The objective. To assess the effectiveness of the selective progesterone receptor modulator ulipristal (Esmya) in presurgicaltherapy of patients with uterine myoma.Patients and methods. The examination included 23 patients with uterine myoma aged 37 to 42 years, 13 patients with uterine myoma aged 21 to 41 years who received Esmya (5 mg/day for 3 months) as a presurgical treatment, 10 patients with uterine myoma aged 33 to 48 years - control group. Removed myoma nodules underwent complex histological examination that included immunohistochemical testing with the use of monoclonal antibodies to estrogen and progesterone receptors; Кi-67; phospho-histone; Вах; Bcl-2; cleaved caspase-3; VEGF.Results. After a 3-month Esmya therapy in 13 patients, the sizes of uterine myoma decreased to correspond to 2-3 wks of gestation, the sizes of the dominant nodule were reduced on the average by 3±0,4 cm in diameter, amenorrhea developed as soon as in the first cycle (complaints of profuse menstrual bleeding stopped). No undesirable effects were noted, the women pointed out to the convenient use of the drug. A morphological examination found large sites of dystrophic changes of leiomyocytes; low-active perivascular growth areas; pseudocapsule sclerosis, single mitotic figures, and marked apoptosis. Analysis of the results of immunohistochemical examination found the following results: phospho-histone and Ki-67 - in single cells; Bax - marked induction; cleaved caspase-3 - distinct expression; VEGF - weak-to-moderate expression; expression of progesterone - 78-87%.Conclusion. As has been demonstrated a decreasing of proliferation and hypertrophy of smooth muscle cells of uterine myoma, under the influence of Esmya; while the surrounding myometrium does not change. The mechanism of the regression effect of Esmya on uterine myoma has been confirmed as a selective tissue-specific «noneffective» receptor protein conformator, inhibiting transcription of progesterone in cells of uterine myoma.

Abstract

Background. Ovarian hyperstimulation syndrome (OHSS) results from increased vascular permeability (VP) caused by ovarian hypersecretion of vascular endothelial growth factor (VEGF), which activates its receptor-2. In animals, the dopamine receptor 2 agonist cabergoline (Cb2) inactivates VEGF receptor-2 and prevents increased VP.Objective. Our objective was to test whether Cb2 reduces hemoconcentration and prevents OHSS in humans.Materials and methods. Cb2 0,5 mg/d (n=65) or a placebo (n=63) was administered from the day of oocyte retrieval (Day 0) until the day of embryo transfer (Day 5). Hemoconcentration were identified markers, such as hematocrit, hemoglobin, red blood cell count, platelet count and indicators of the inflammatory response of leukocytes.Results. Hematocrit (p<0,0001), hemoglobin (p<0,0001) were significantly lower on day 5 after treatment with Cb2 as compared with placebo. The incidence of OHSS was 7 (10,7%) and 19 (30,1%) respectively (p=0,0315). This reduces the risk (ARR) of complications absolute best 19% [CI 5-33%], and decrease the likelihood of ovarian hyperstimulation syndrome (OR) in 3,6 times [CI 1,3-9,3].Conclusion. Given that Cb2 is a well - established and safe medication, this study provides proof of concept for the use of dopamine agonists in the prevention of OHSS in women undergoing assisted reproduction. The preventive effect of selective dopamine D2-receptor agonist, cabergoline during the prevention of ovarian hyperstimulation syndrome manifests a decrease in the degree of hemoconcentration as one of the major pathophysiological stages of the syndrome.

The early complex correction of thrombophilia in patients with intrauterine growth restriction

Abstract

The actual direction of modern public health service is the reduction of the level of diseases in pregnancy. Hypercoagulable state is one of the major causes of complications and often associated with intrauterine growth restriction. The purpose of this study was to determine whether early complex correction of thrombophilia, including vitamins (folic acid, Vitrum Prenatal Forte) and anticoagulation therapy reduce risk of intrauterine growth restriction and pregnancy loss. Materials and methods: 30 pregnant patients with intrauterine growth restriction of different severity level and 30 healthy pregnant women. Thrombophilia of different causes was detected in 60% intrauterine growth restriction. We conclude that the methods of early diagnostics and complex correction of hypercoagulable state in patients with intrauterine growth restriction significantly decrease perinatal morbidity rates.

Abstract

With the help of innovative research methods (such as probing and scanning electronic microscopy, transmission microscopy) the examination of placenta, umbilical cord, endometrium, myometrium and maternal blood for the presence of severe preeclampsia was held. It was shown that poor circulation in the structural components of the studied tissues and organs, as well as changes in cytoarchitectonics and functional properties of red blood cells may be a component of ICE launch - a syndrome with subsequent breach of maternal and fetal life. Application of dipyridamole (Curantil®) improves maternal-placental blood flow, increasing the risk of weakening the blood coagulation and helping to improve blood flow to the capillaries of the placenta and myometrium.

Abstract

He aim of the study was to examine the peculiarities of gestation and birth for asymptomatic bacteriuria (AB) and to determine the efficacy of antimicrobial therapy in combination with the use of proanthocyanidins. Results: complications during and after childbirth occurred 15 times more often in women with low AB (102-104 CFU/ml) treatment which was not carried out , and 2 times more frequently in the patients with the release of ≥ 105 CFU/ ml and held antibacterial treatment than in the control group. Every sixth pregnant with AB was treated only with antibacterial drugs (8 times more often than in the group of women taking antibiotics in combination with proanthocyanidins) there was a lack of resistant bacterial response.
