Abstract

Modern views on the issue of menopausal hormone therapy based on updated recommendations of the International Menopause Society 2013 are reviewed in the following article as well as the basic principles of hormone therapy in peri - and postmenopausal women, its benefits and possible risks. An important emphasis is made on unequal undesirable consequences from the use of combined hormonal preparations, depending on their composition and, above all, progestin component.

Abstract

In this paper we investigate the effect of dienogest on endometrial morphology and function in patients with external genital endometriosis. It is shown that dienogest is effective in reducing pain in patients with external genital endometriosis and endometrial morphology and function improves, suppresses manifestations of inflammation in the lining of the uterus.

Micronized progesterone: several aspects of miscarriage prophylaxis

Abstract

Progesterone is an essential hormone in the process of reproduction. Progesterone offers an effective intervention when the continuation of pregnancy is at risk from immunological factors, luteinic and neuroendocrine deficiencies, and myometrial hypercontractility. Progesterone has been successfully used as prophylaxis in the prevention of spontaneous miscarriage, with treatment beginning from the first trimester of pregnancy. There is substantial evidence, too, to indicate that women with idiopathic recurrent miscarriage may benefit from the immunomodulatory properties of progesterone in early pregnancy. The use of progesterone has been extensively studied in the prevention of preterm birth in a variety of settings. This paper reviews the evidence for the safety and efficacy of the use of progesterone in each of these indications.

High frequency of unplanned pregnancies remains a significant problem in many countries, including Russia. This figure can be significantly reduced when the reversible contraceptive long-acting methods are used (Long-Acting Reversible Contraception - LARC), including intrauterine system (IUS). LARC-methods are highly effective, and are mainly characterized by small number of contraindications and side effects, thus being economically profitable along the application. Particular attention is paid to the benefits of levonorgestrel-releasing system, which, if timely advised can be a good choice for most women.

Quality of life of women with premenstrual syndrome in dynamics of nonhormonal treatment and hormonal therapy

Abstract

PMS may have a negative impact on the daily lives of women, leading to various disorders. Objective: based on the survey results with general international questionnaire MOSSF-36 in order to examine the quality of life (QOL) of patients with PMS ranging from average to severe, before and after treatment with herbal remedies, on the basis of dry extract of Vitex agnus castus and oral hormonal contraceptives (COC) containing 20 mcg of ethinyl estradiol and 3 mg of drospirenone in a 24+4 regimen.We observed 243 patients aged 16 to 45 years, of mean age of 29,7±5,8 years, and an average degree of heavy PMS has been identified and confirmed by ICP-diaries. To assess the quality of life and ICP influence on the physical and psychological aspects of life in patients the late luteal phase of the menstrual cycle twice, before and after treatment, filled the general quality of life questionnaires Medical Outcomes Study - Short Form (MOSSF-36). After treatment the statistically significant decrease in deformation parameters, both physical and psychological components of health was shown.Conclusion. Treatment of women with PMS and the average degree of severe type with herbal remedies, on the basis of dry extract of prostrata ordinaryextract and COC containing 20 mcg of ethinyl estradiol and 3 mg of drospirenone in 24+4 regimen has led to the improvement of the patients, as well as their quality of life, which resulted in a statistically significant decrease in strain indicators of physical and psychological components of health.

The influence of biopharmaceutical Klimalanin on the secretory synthetic kinesis of the blood monocytes/macrophages

Abstract

The condition of stability of lysosome membranes of blood monocytes and peritoneal macrophages and their secretory synthetic arousal of 23 women with vasculomotor affect of climacteric syndrome were investigated. It was educed that in comparison with healthy women, these women had the labialization of monocyte’s lysosome membranes and peritoneal macrophages, enhancement of the secretion of the intercellular lysozyme and decrease of its fusion. Usage of the Klimalanin as a medication leads to the stabilization of the lysosome membranes of these cells, decrease of fusion in it and increase of lysozyme secretion.

Cytomegalovirus infections: therapeutic potential in pregnancy

Abstract

The article presents the results of simultaneous clinical multicenter controlled study of comparative assessment of therapeutic efficacy, safety and tolerability Panavir, solution for intravenous injection of 0,04 mg/ml with the Standardl human immunoglobulin, intravenous solution in the complex treatment of cytomegalovirus infection in pregnant women in the II and III trimester. It was found that introduction of Panavir complex therapy effectively reduces acute cytomegalovirus infection titer immunoglobulin G and prevents infection of newborns, making it safe for both mother and fetus, and is not accompanied by side effects and adverse reactions.

Differentiated approach to the treatment of various forms of vulvovaginal candidiasis

Abstract

The article presents data on the results of examination and treatment of 55 patients with a diagnosis of «vulvovaginal candidiasis». In the study, the analysis of anamnestic data, and in addition, the clinical evaluation of vaginal discharge and the condition of the mucous vulva and vagina, determining pH of the vaginal environment, microscopic examination of vaginal smear, extended colposcopy is presented.After verification of the diagnosis 18 (32,73±6,33%) patients with acute uncomplicated vulvovaginal candidiasis were treated for local forms of antifungals (butokonazol), 37 (67,27±6,33%) patients with chronic complicated form of vulvovaginal candidiasis - local therapy (butokonazol), and then the system supports, suppressive therapy with fluconazole.Our results indicate that use of the drug Ginofort® (butokonazol) in patients with both acute and chronic form of vulvovaginal candidiasis yielded reliable positive therapeutic effect (p<0,001).

Physical aspects of photodinamic therapy application to chronic vulvovaginal candidiasis

Abstract

The aim of the presented research was investigation of induced by 5-ALA PPIX accumulation kinetics in mucosa, vagina and vulva chronic in women with vulvovaginal candidiasis after treatment of 5-ALA and the choice of optimal time mode of fluorescence diagnosis and photodynamic therapy (PDT) in chronic vulvovaginal candidiasis. The investigation was held in group of 60 women: 20 with vulvovaginal candidiasis, 25 - bacterial vaginosis, 15 - non - specific vaginitis. Each woman had 4 sessions of PDT. Before and after each of the sessions fluorescence diagnosis (FD) of photosensitizer accumulated in mucosa, vagina and vulva was performed. By means of local fluorescent spectroscopy difference of accumulated PPIX was estimated with statistical methods. It was obtained, that difference between accumulation of PPIX after 1, 2, 3 and 4th session is statistically significant. After the 4th session it was estimated that there is no significant statistical difference between PPIX accumulated in healthy tissue and mucosa of vagina. According to this we concluded, that PDT in this case showed high efficiency.

E7 oncoprotein as a marker for persistence of human papillomavirus infection in patients with chronic cervicitis

Abstract

A comparative analysis of the results of clinical, gynecological and laboratory diagnostic methods of examination in 135 patients with chronic cervicitis. Patients were divided into 3 groups. Group 1 included 49 women with chronic cervicitis on the background of human papillomavirus types 16 and 18 in the cervical canal. In 12 patients of group I revealed the presence of E7 protein. Group 2 consisted of 32 patients with chronic cervicitis and human papillomavirus types 11 and 31, 33, 35, 39, 45, 51, 52, 56, 58, 59, 6 in the cervical canal. The group 3 included 54 patients with chronic cervicitis without the presence of human papillomavirus infection in the cervix.

Lactobacilli play an important role in maintaining the normal vaginal biocenosis. In patients with chronic inflammatory diseases of the urinary tract above the risk of nonspecific vulvovaginitis. Inclusion of oral probiotics in the treatment regimen of inflammatory diseases of the urinary tract can significantly improve outcomes and reduce the number of relapses.

Rational approaches to the prevention of vascular complications in patients with endometrial hyperplasia

Abstract

Endometrial hyperplasia, especially against the background of the metabolic syndrome are tromboopasnosti state, especially in the appointment of hormone replacement therapy significantly increases the risk of thrombosis.Screening for thrombophilic states (genetic defects in the system of blood coagulation and fibrinolysis) helps to identify patients at risk and make appropriate adjustments in tactics such patients. The study involved 207 patients having had 10 different genetic defects in hemostasis and the data obtained allowed to approach the treatment of patients with new positions.In the presence of cardiovascular disease and, based on data and hemostasiogram agregatogrammy, patients with endometrial hyperplasia, in addition to hormonal therapy (triptorellin and/or dydrogesterone) administered dipyridamole, which has antiplatelet and vasodilatory action, reducing the ability of platelets to aggregate reduction the risk of thrombosis and improve the quality of life in our cohort of patients surveyed.

As a consequence of this, there is a fluid shift into third-space causing hemoconcentration, it may contribute to hypercoagulable state, with increased risk of venous thromboembolism. Abstract

The thin endometrium often turns out to be the cause of the considerable decrement of the pregnancy after the IVF procedure. It is considered that the endometrium thickness which is less than 7 mm (in the implantation window) provides the minimal chance for the efficient conception. Up to the present time in the modern scientific literature we did not found any facts about the treatment of the patients with the thin endometrium by means of the gas mixture (CO2 and N2) processing of the endometrium, which provides the considerable blood circulation enhancement combined with gynecological massage and, as a result, the rising of the thickness of the basal and functional layers. We are recommend this method as a preparing women for IVF.

Swelling in pregnant women: a disease or a norm?

Abstract

During pregnancy the effects of a number of predisposing factors (influence of progesterone, a violation of the passage of urine, the occurrence of vesicoureteral reflux) often lead to exacerbation of chronic diseases of the urinary tract, the development of eye and preeclampsia. This has an adverse impact on the course of pregnancy and perinatal outcomes. The Multicenter study «Evaluation of efficacy, safety and tolerability of cranberry extract in soft capsules Iuravit (biologically active additive to food) for prophylaxis and complex therapy of urinary tract infections in women during pregnancy» in cases of 195 women showed high efficacy, tolerability, a small number of side effects (5,6% of women), and worsening of urinary tract infections in 4,6% (mostly due to a violation of the outflow of urine and indications for stenting ureters). It allows to recommend the use of cranberry extract Juravit as prophylaxis in pregnant women with diseases of the urinary tract.
