Antiviral therapy for recurrent genital herpes

Abstract

Herpes infection is one of the most common viral infections in man and presents a serious medical and social problem. In the Russian Federation, the incidence of genital herpes in 2012 amounted to 16.8 per 100 thousand population. The article deals with questions of clinic, diagnostics and applications of antiviral drug with immunomodulatory effects in the treatment of recurrent genital herpes.

Vaginal atrophy in obstetrician-gynecologist practice

Abstract

Vaginal atrophy is a characteristic symptom of menopause, which can develop in young women as a result of temporary or permanent deficiency of estrogen, as well as after operations of the cervix or connective tissue diseases. Manifestations of diverse vaginal atrophy, affect sexuality and significantly reduce the quality of life of women. The diagnosis of vaginal atrophy is clinical and depends solely on the skill of the doctor who expertly evaluates medical history and examines, since patients with vaginal atrophy often do not complain. Estrogens are considered to be the most effective option for the treatment of all forms of vaginal atrophy in the local application. In Russia, it is only estriol - estrogen deprived of systemic metabolic effects and does not require monitoring of the endometrium and breast, as well as the additional use of progestogen to protect the endometrium - that is registered for the vaginal use. Estriol in the form of vaginal suppositories (Ovipol Clio®) can be used in women with urogenital atrophy in postmenopausal women with amenorrhea or in the reproductive age, in the treatment of stenosis of the cervical canal, or after unclear smear from the cervix to the background of vaginal atrophy, as well as in urology in relapsed urinary tract infection in young women or as a preoperative preparation for operations on the pelvic floor.

Clinical management of patients with diseases of the cervix with persistent HPV infection: the possibility of combined treatment

Abstract

Understanding the persistence of HPV infection in cervical epithelium and associated development of benign diseases defines the tactics and treatment of cervical lesions associated with HPV.The Aim. To study the efficacy and long-term results of treating the benign cervical disease with persistent HPV infection when using the drug in combina-tion with Isoprinosine surgically.Material and methods: 80 patients with persistent HPV infection and cervical diseases. Combination therapy included: antiviral treatment of HPV infection (inosine pranobex) treatment aimed at the elimination of concomitant opportunistic microflora, yeasts and other microorganisms, radio frequency destruction, restoration of vaginal biocenosis. Determining factor in the choice of surgical intervention were the following factors: cytological and colposcopic data research methods; the presence or absence of a pathological process involving the cervical canal.The Results. Combination antiviral therapy in conjunction with radio wave surgery of benign cervical disease shows patients with persistent HPV infecion, have LSIL and HSIL, quantitative test positive for HPV, and had undergone previous surgical treatment of cervical other methods.

Treatment of urogenital mixed infection in women of reproductive age: current state of problem

Abstract

An open, randomized clinical and immunological and microbiological study of 102 women of reproductive age with inflammatory diseases of the genitourinary system associated with opportunistic microorganisms has been held. It has shown that the presence of genital tract Mycoplasma hominis, Ureaplasma urealyticum, fungi of the genus Candida is accompanied by impaired innate immune factors, changes in the qualitative and quantitative composition of the resident microflora. Application in the complex therapy of the drug in the form of polyoxidonium suppositories at a dose of 6 mg daily for 10 days promotes a more rapid resolution of symptoms of inflammation, eliminating the imbalance in the local immune status, and thus leading to restoration of colonization resistance compared to baseline therapy.

Estrogen monotherapy in women with surgical menopause

Abstract

Surgical menopause is a pathology of women of reproductive age, developing after surgical removal of the ovaries to the uterus or without a uterus. Severe estrogen-deficient state has been progressing for 1-2 days after surgery, and therefore the hormone replacement therapy should be initiated in the first days and weeks after surgery. In women with postovariectomy syndrome and hysterectomy an unopposed estrogen monotherapy may be conducted. When expressed vegetative-vascular symptoms of menopause after surgery recommended the appointment of estriol (Ovestin) 2 mg orally 2-4 times per day (up to 8 mg) for 4 weeks. After reliever dose is gradually reduced to 1-2 mg/day. As a rule, women with hysterectomy may be administered estrogen alone without the addition of progestogen for 5 years or more.

Evaluation of saturation of pregnant women with vitamin D in the application of different doses of сolekalciferol

Abstract

This article presents the results of a randomized prospective controlled study assessing the saturation of pregnant women with vitamin D deficiency with the original difficiency or a lack of 25(OH)D, the first blood sampling and randomization was produced at 12-16 weeks. two groups of women who have been daily recieving colecalciferol in doses of 500 IU (Group 1, n = 45) and 2000 IU (group 2, n = 45) from first trimester until delivery was analyzed . Initial average level of 25(OH)D in all pregnant women had no significant difference (16,45 ng/ml and 16,5 ng/ml, respectively). None of women of first group reached the final level of 25(OH)D corresponding to the value of the norm; 95,55% of patients in group 2 (n=43) reached the level of serum 25(OH)D 30 ng/ml (mean level of 39,11 ng/ml). Research has shown the efficacy of colecalciferol intake in doses of 2,000 IU, which may be recommended for pregnant women with 25(OH)D serum <30 ng/ml.

Infertility and ovarian endometriosis patients: treatment efficacy

Abstract

The study included 59 patients of reproductive age with ovarian endometriosis and infertility. Depending on differentiated tactics, taking into account the availability of relapsing forms of ovarian endometriosis, ovarian reserve status, size endometrioma, patients were divided into 2 groups. In group 1 (n=13) conducted an empirical therapy, in group 2 (n=46) - surgery followed by hormone therapy, pregnancy rate after treatment was assessed at 6 and 12 months, respectively. In infertile patients with endometriosis ovarian pregnancy rate after completion of empirical therapy and two-step regimen was 30,8 and 43,5%, respectively.

Assessment of the drug efficacy for treatment of cervical intraepithelial neoplasia grade I-II with pharmaceutical composition Cervicon-DIM in suppository form (Interim results of a clinical trial)

Abstract

The article presents the results of the clinical study about the effictiveness and safety of the novel pharmaceutical composition Cervicon-DIM in suppository form containing 3,3’-diindolylmethane (DIM) substance in 3 months course of conservative treatment of 78 women childbearing age with CIN I-II. The study was multicenter randomized placebo-controlled double-blind and conducted in parallel groups. We observed efficacy of the therapy of patients with CIN I-II by intravaginal application of the Cervicon-DIM in a dose of 200 mg DIM per day. Complete regression of cervical lesions by the histologic examinations was for 85% women.
